DeKalb High School Return to Play

Prior to Participation- All Athletes must have a new Physical on file or if using last year’s Physical, then the additional Health History Update Questionnaire and Release Consent Form, an updated Concussion/Cardiac Form, and The DeKalb Central Waiver/Release Form should be completed and turned in to the Athletic Office.  All forms should be turned in by July 6th for athletes to begin training.

These forms can be found on the DeKalb High School Athletics Webpage or can be picked up inside Door 1 of the High School during regular office hours. Monday-Thursday from 8:00 AM-3:30 PM and Friday from 8:00 AM-11:30 AM.

Athletes/Coaches/Trainers who are experiencing any of the following symptoms should not attend any workouts/practices and should remain at home: fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, or diarrhea.

Athletes/Coaches/Trainers will be given a self-check questionnaire when arriving each day that must be completed.  If “Yes” is answered to any of the above symptoms, then that Individual will be sent home.  Communication with parents, school administration, trainer/nurse will be done immediately.

-Athletes/Coaches/Trainers who are sent home will need to be cleared in writing by a Physician before returning.

-Athletes/Coaches/Trainers who are in the high risk category with preexisting conditions should not attend any workouts/practices or accepts all risk from attending workouts/practices.

-Athletes/Coaches/Trainers will be trained on Social distancing, how and where to wash hands or find hand sanitizer, and disinfect all equipment after use.  All Athletes should plan to provide their own water bottle for any practice that they attend.

-Because of ongoing concerns with the Covid Virus, changes can be made to this document or all schedules at any time in accordance from the DeKalb County Health Department or DeKalb Central Schools.
